About The Position
Pinnacle Home Care is hiring a PRN Licensed Practical Nurse (LPN) to provide compassionate, patient-centered skilled nursing care in patients’ homes. The nurse works under RN supervision and carries out care-plan tasks while using clinical expertise to support positive patient outcomes. The role involves timely and accurate electronic medical record documentation, coordinating daily visits, and following agency policies and quality, complaint-management, patient-rights, and discharge procedures.
Requirements
- Current LPN or LVN license applicable to the state.
- Graduate of an accredited nursing program.
- Valid BLS certification.
- At least one year of nursing experience.
- Excellent communication, organizational, and time-management skills.
- Valid driver’s license and access to a reliable, insured vehicle.
- Ability to visit homes that may not be wheelchair accessible.
- Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
- Ability to walk, stand, stoop, and occasionally lift or turn patients.
Nice To Haves
- Home health or medical/surgical nursing experience preferred.
Responsibilities
- Administer skilled nursing care under RN supervision.
- Perform curative, rehabilitative, or preventive care tasks outlined in patient care plans.
- Use clinical expertise and nursing education to make informed, independent decisions that support patient care and positive outcomes.
- Complete visit reports and all required documentation promptly and accurately in the electronic medical record system.
- Confirm daily visit schedules to coordinate care with other staff.
- Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
- Follow the agency’s process for managing and addressing customer complaints.
- Uphold patient rights in accordance with agency service agreements and admission policies.
- Follow agency policies regarding appropriate patient discharge and reasons for discharge.
