Total Extended Care Services, a growing Physical Medicine and Rehab group is looking for a dedicated Nurse Practitioner/Physician Assistant to join our team. In this role, you will partner with our PM&R physician in the sub-acute rehab setting. This position is perfect for those passionate about improving patient outcomes and working within an interdisciplinary, specialized team. About the Role: Location : In-person at skilled nursing facilities Schedule : Flexible options, Part-time position available Culture : Detail-oriented, outcome-focused, stable, people-oriented, and team-oriented
Requirements
- Licensed Nurse Practitioner (Required)
- Certified Nurse Practitioner (Required)
- Licensed Physician Assistant (Required)
- Certified Physician Assistant (Required)
- Must be licensed in TN
- Reliable transportation
Responsibilities
- Partner with the facility's therapy department to provide the best possible rehab care.
- Communication and collaboration with the patient's primary care team to ensure the best outcomes.
- Timely charting/documentation is required but can be done from home.
