About The Position

Requirements

• For APRN/NP applicants: Master of Science in Nursing (MSN).
• For APRN/NP applicants: current, unrestricted Nurse Practitioner license and certification through a national certifying body.
• For APRN/NP applicants: certificate of completion from an accredited Nurse Practitioner program.
• For APRN/NP applicants: must have or be eligible to obtain a Nevada Pharmacy and DEA License.
• For PA applicants: master’s degree in Physician Assistant Studies from an accredited Physician Assistant program.
• For PA applicants: National Commission on Certification of Physician Assistants (NCCPA) certification.
• For PA applicants: Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification.
• Must be certified through a national certifying body.
• Must have or be eligible to obtain a Nevada Pharmacy and DEA License.
• Must maintain all required certifications and licensures.
• Minimum of two years of professional APRN, PA, or NP experience in a clinic setting.
• Minimum of three years of supervisory experience.
• Valid driver’s license.
• Knowledge of health promotion, health risk identification, and patient education principles and techniques.
• Knowledge of community, professional, and educational resources.
• Knowledge of practice management systems, including electronic health record systems (EHRs).
• Ability to competently use Microsoft Office, including Word, PowerPoint, and Excel, and appropriate practice management software.
• Skills in case management, time management, problem solving, crisis intervention, multitasking, and other organizational matters.
• Ability to establish and maintain effective working relationships with patients, clinical staff, and the public.

Nice To Haves

• Experience in primary care, Internal Medicine, or General Family Medicine.

Responsibilities

• Provide patient care in an outpatient setting and clinical services as assigned.
• Provide acute care, hospital follow-up, and independent patient care.
• Conduct wellness and transitional care management (TCM) visits.
• Assess patient status using health histories, patient or family interviews, chart reviews, illness assessment, risk factors, family history, psychosocial and cultural factors, and physical examinations.
• Communicate case management information to other professionals and community agencies.
• Order or perform appropriate laboratory, diagnostic, and screening tests.
• Seek additional information and consult physicians or other clinicians as needed to evaluate illness.
• Integrate clinical information to determine diagnoses and therapeutic plans, including identifying health risks.
• Develop and implement treatment plans, including prescribing or dispensing medications and injections in compliance with medical practice guidelines and state laws.
• Handle minor lacerations.
• Instruct patients and families regarding medications and treatments, and educate them about health promotion and illness prevention.
• Recommend community resources to meet patient and family needs.
• Support inbox management for the General Internal Medicine Team, including physicians.
• Support quality measures, including HEDIS metrics for Medicare, Medicaid, and Managed Care.
• Prepare medical record documentation, including examination and test results, diagnoses, medications, and treatments.
• Participate in peer reviews, chart reviews, staff education, clinical guideline development, continuing education, and quality assurance activities.
• Serve as a consultant or collaborator to the clinical team in the provider’s specialty area.
• Comply with patient confidentiality requirements and promote patient advocacy.
• Collaborate with physicians in managing patients’ acute and long-term medical needs.
• Provide monitoring and continuity of care between visits according to treatment plans, including triaging patient calls and emails.
• Collaborate with multidisciplinary team members and make appropriate referrals.
• Perform other duties as assigned.
