About The Position

This role functions as a liaison between internal/external referral sources and inpatient rehabilitation operations. The Clinical Rehab Navigator will establish relationships and aid with communication to case managers, physicians, payors, patients, families, and referral sources to generate referrals and drive patient admission volume in the inpatient rehab. This role ensures appropriate patient selection through clinical chart assessments during on/off-site clinical evaluation of patients in collaboration with the physician. The navigator will also complete in-service and educational opportunities for physicians, case managers, interdisciplinary members, and other professionals, serving as an educational resource to patients, family members, and caregivers. Additionally, this role participates in the development and implementation of business plans for the rehab unit and contributes to the development and maintenance of referral source promotional information.

Requirements

  • Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date.
  • Licensed Rehab Therapist specializing in Physical Therapy Asst credentialed from the Texas Board of Physical Therapy Examiners obtained prior to hire date or job transfer date.
  • Rehab Therapist specializing in Occupational Therapy credentialed from the Texas Board of Occupational Therapy Examiners obtained prior to hire date or job transfer date.
  • Certified Rehab Therapist specializing in Occupational Therapy (COTA) credentialed from the Texas Board of Occupational Therapy Examiners obtained prior to hire date or job transfer date.
  • Licensed Respiratory Therapist credentialed from the National Board for Respiratory Care (NBRC) obtained prior to hire date or job transfer date.
  • Licensed Clinical Social Worker credentialed from the Texas State Board of Social Worker Examiners obtained prior to hire date or job transfer date.
  • Speech-Language Pathologist credentialed from the Texas State Board of Examiners for Speech-Language Pathology and Audiology obtained prior to hire date or job transfer date.
  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Nice To Haves

  • BLS Provider preferred. American Heart Association or American Red Cross accepted.
  • Drivers License preferred.
