Requirements

• Active Florida RN or LPN license
• Strong communication and interpersonal skills
• Detail-oriented and organized
• Microsoft Office proficiency
• Valid driver’s license and reliable transportation

• Knowledge of payer sources and referral processes

Responsibilities

• Build relationships with case managers, social workers, physicians, and hospital staff within the Flagler Hospital and St. Vincents-St. Johns Hospital Systems.
• Educate referral sources on home health services, programs and eligibility, and Concierge Home Care offerings.
• Support referral growth and admissions goals.
• Meet patients at the bedside before discharge to explain home health services and benefits.
• Educate patients and families on insurance coverage, eligibility, and skilled home care services.
• Advocate for safe and seamless discharges.
• Coordinate care from hospital to home, collaborating with DME providers, infusion companies, PCPs, and caregivers.
• Support patients during critical post-discharge periods.
• Help reduce avoidable readmissions.
• Partner closely with intake teams, clinical managers, schedulers, and account executives.
• Provide timely referral and patient updates.
• Ensure continuity of care throughout the admission process.
• Process referrals in real time.
• Complete admission documentation accurately and in a timely manner.
• Track referral activity, conversion rates, and admission barriers.
