Our Orthopedic Surgery specialists provide care for a broad spectrum of bone and joint injuries, treating conditions affecting ankles, feet, hands, shoulders, elbows, and spine, as well as musculoskeletal tumors. The RN Care Coordinator will coordinate care across the health care continuum within designated sub-specialty population(s). They will serve as a single point of contact by facilitating and coordinating transitions in care within and between care settings, providers, and services. This role also involves completing pre-visit planning and post-visit wrap-up, determining vital follow-up appointments and/or treatments to optimize patient visits, and facilitating proactive disease management and preventative health care using evidence-based practice. Additionally, the RN Care Coordinator may room patients for the surgeon(s) as needed, complete clinic readiness including records review, and provide administrative support.
