The RN Assessment Coordinator is responsible for assessing and determining the health status and level of care for all new admissions. This role ensures the accurate and timely completion of all MDS Assessments, including PPS Medicare, quarterly, annual, and significant change assessments. The coordinator communicates the level of care for new residents to all disciplines and coordinates interdisciplinary participation in completing the Minimum Data Set (MDS) according to regulatory time frames. They are responsible for ensuring the completeness and thoroughness of documentation as mandated by federal, state, and medical standards, including data entry and electronic submission of MDS assessments, verification, corrections, and record maintenance. Additionally, the RN Assessment Coordinator schedules and conducts resident care conferences, assists in formulating and revising care plans, evaluates their comprehensiveness and individuality, and assesses the achievement of desired outcomes. This role also manages all level of care changes within the facility, notifying all departments and generating/transmitting appropriate forms to agencies as required by state law.
