As a Patient Engagement Coordinator, you’ll be one of the first faces and voices of Town Square Health, meeting prospective patients in the community, following up with warmth and consistency, and making sure that the path from “I’m interested” to “I have an appointment” is as smooth as possible. You’ll play a meaningful role in building our patient panel from the ground up, and your work will directly shape how people in the Rochester community experience us for the very first time. The right person for this role brings genuine enthusiasm for connecting with people, a natural instinct for hospitality, and the follow-through to make sure no one falls through the cracks. Experience in healthcare is helpful, but a deep commitment to customer service and a passion for serving older adults will take you far here. You’ll have the opportunity to: Be a trusted first point of contact for Medicare-eligible patients and their families Represent Town Square Health in the Rochester community at outreach events and health fairs Help shape what a welcoming, patient-centered experience looks like as we grow Work alongside a collaborative team that is genuinely invested in doing healthcare differently
