About The Position
Heritage Home Health and Hospice is seeking a Registered Nurse to provide hospice care in patients’ places of residence. The RN works with patients, their families, and other healthcare professionals, performs visits, coordinates care, and serves as the point of contact for all disciplines involved in providing patient care. The RN follows agency, state, and federal policies and procedures, consults with patients’ primary care physicians, ensures medication profiles are accurate, and attends meetings as required. Heritage is locally owned and operated and provides individualized home-based care to people recovering from illness, injury, or surgery, living with chronic disability, or experiencing aging-related physical or cognitive conditions. Requirements
- Currently licensed as a Registered Nurse in the state of Idaho.
- Demonstrate knowledge and skill in nursing practice.
- Hold professional CPR certification.
- Have general computer knowledge and the ability to learn industry-specific software and documentation applications.
- Possess a valid state driver’s license and automobile liability insurance.
Nice To Haves
- A minimum of one year of clinical experience is preferred but not required.
Responsibilities
