About The Position
The Registered Nurse provides home health care to patients in Elk Grove Village, Illinois, through Ascension Home Health’s Home Health Therapeutic Services department. The role delivers direct care, including triage, blood draws, and wound management, while maintaining high-quality clinical records. The nurse develops personalized care plans with the medical team, coordinates care across disciplines and discharge planning, and provides health education to patients, caregivers, and staff. The position includes travel to local homes and care facilities and is scheduled for 20 hours per week on the day shift.
Requirements
- Hold an Illinois Registered Nurse credential from the Illinois Board of Nursing before the hire or job transfer date.
- Hold an Illinois driver's license before the hire or job transfer date.
- Obtain BLS Provider certification within 30 days of hire or job transfer; American Heart Association or American Red Cross certification is accepted.
- Have a diploma from an accredited school or college of nursing, or the required professional licensure at the time of hire.
- Have at least 2 years of hospital or home health experience.
Responsibilities
