About The Position
WelbeHealth is seeking a licensed Nurse Practitioner or Physician Assistant to provide community-based, comprehensive care to vulnerable seniors through its value-based PACE program. The role offers the opportunity to build long-term relationships with a smaller patient population and work as part of a mission-driven interdisciplinary team. Providers deliver care through WelbeHealth’s California clinics and home visits, with regular hours of 8:00 a.m. to 5:00 p.m., Monday through Friday, and a rotating phone-only on-call schedule a few nights or weekends per month. Compensation consists of base salary plus bonus, with an advertised range of $193,606.40–$231,192. Actual compensation depends on experience and relevant qualifications. The position requires compliance with WelbeHealth’s vaccination policy: proof of vaccination or a vaccine declination form is required before employment. The organization also maintains infection-control and PPE standards.
Requirements
- Licensed Nurse Practitioner or Physician Assistant.
- At least two years of experience working with elderly patients.
- Prescriptive authority.
- DEA registration.
- Eligible to serve Medicare/Medi-Cal participants.
- Obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter.
- Comply with WelbeHealth’s vaccination policy by providing proof of vaccination or a vaccine declination form before employment.

Responsibilities
- Provide comprehensive, holistic primary care to vulnerable seniors in the PACE program.
