About The Position

Central City Concern is hiring a Primary Care Float Provider (NP, PA, MD, or DO) to provide patient-centered primary care across multiple clinical and community settings. The role has no assigned patient panel and floats among its home bases—Old Town Clinic Walk-In Clinic and Blackburn Center—and other sites, including Evergreen/Recuperative Care Program, Hansen Clinic, and Mobile Health. Care settings include walk-in and urgent care, wound care, addiction services, medical respite, shelter-based care, and mobile community care. The provider’s work may include acute illness treatment, wound management, recovery support, and chronic disease care. The provider works as part of a multidisciplinary primary care team, while also practicing independently with medical and administrative support. The role coordinates care with programs such as Hepatitis C treatment, withdrawal management, residential substance use treatment, specialty mental health, and overdose prevention. It also contributes to clinical workflow development, quality improvement, provider education, and onboarding. Scheduled after-hours phone coverage is shared on rotation with colleagues; calls are first addressed by a contracted nurse triage service, with licensed medical providers as the second call. Applicants must meet Central City Concern privileging requirements and role-specific licensure and certification requirements. The position requires full prescriptive powers in Oregon with a DEA number and current BLS certification. Candidates must pass pre-employment drug screening, TB testing, and background checks, including DHS Background Check Unit clearance. The role may require work at other CCC clinical sites or off-site community events. Employees must commit to at least 20 hours (0.5 FTE) per week, with scheduling flexibility available for those seeking less than full-time hours.
