About The Position

HarmonyCares is hiring a Geriatric Advanced Primary Care Provider (NP/PA) to provide value-based, in-home primary care for frail Medicare and Medicaid patients with complex healthcare needs. The role focuses on managing chronic conditions and delivering personalized, patient-centered care under an integrated, team-based, physician-driven model.

Providers deliver primary care to high-acuity patients in their homes Monday through Friday, with an emphasis on taking the time needed to address patients’ needs rather than focusing on volume. The role has no holidays or hospital rounds and minimal call responsibilities. An appointment assistant and a company car with driver are available in select markets.

Requirements

• Licensed as an NP or PA in the state where the HarmonyCares Medical Group office is located.
• Federal DEA license or eligibility.
• Active CPR/BLS certification.
• Board certification.
• Valid, unencumbered driver’s license and good driving record.

Nice To Haves

• Geriatric training or experience is preferred.
• Directly related experience in home health, geriatrics, and primary care.

Responsibilities

• Provide in-home primary care to high-acuity, frail Medicare and Medicaid patients.
