Advance Practice Provider (NP /PA) Value-based Primary Care
About The Position
HarmonyCares is a national value-based provider of in-home primary care for people with complex healthcare needs. The organization operates home-based primary care practices in 14 states and employs more than 200 primary care providers delivering patient-centered care through an integrated, team-based, physician-driven model. The Nurse Practitioner provides primary care to high-acuity patients in their homes, with a focus on patient care rather than visit volume. Clinicians are expected to take the time needed to address each patient's needs. The role is Monday through Friday, with no holidays or hospital rounds and minimal call responsibilities. Appointment support and a company car with a driver are available in select markets.
Requirements
- Licensed as a Nurse Practitioner in the state where the HarmonyCares Medical Group office is located.
- Federal DEA license or eligibility.
- Active CPR/BLS certification.
- Board certification.
- Valid, unencumbered driver's license with a good driving record.
- Directly related experience in home health, geriatrics, hospice, and primary care is sought.

- Geriatric training or experience preferred.
