Advance Practice Provider (NP/ PA), Value-based Primary Care
About The Position
HarmonyCares is seeking an Advance Practice Provider (NP/PA) to manage chronic conditions for a high-risk geriatric patient panel. The role provides value-based, in-home primary care to high-acuity patients Monday through Friday, emphasizing patient care rather than visit volume and allowing clinicians time to address each patient’s needs. HarmonyCares is a national value-based provider of in-home primary care for people with complex healthcare needs. It operates home-based practices in 14 states and employs more than 200 primary care providers in an integrated, team-based, physician-driven model. Its mission is to bring personalized, quality-based healthcare into the homes of patients who have difficulty accessing care. The role includes minimal call responsibilities and no holidays or hospital rounds. In select markets, an assistant may support appointments and a company car with a driver may be available. Compensation packages are based on factors including skill set, experience, qualifications, and other job-related considerations.
Requirements
- Licensed as a Nurse Practitioner or Physician Assistant in the state where the HarmonyCares Medical Group office is located.
- Federal DEA license or eligibility for one.
- Active CPR/BLS certification.
- Board certification.
- Valid driver’s license and a good driving record.
Nice To Haves
- Directly related experience in home health, geriatrics, hospice, and primary care.
- Geriatric training or experience.
