Advance Practice Provider (NP/PA) - Value-based Primary Care
About The Position
HarmonyCares is hiring a geriatric Advanced Practice Provider (NP or PA) for a value-based, in-home primary care role. The position serves high-acuity patients in their homes across the Denton, Aubrey, Lewisville, and Carrollton areas. Clinicians typically see approximately 6–9 patients per day, with 30–45 minutes allotted for each visit, and are encouraged to focus on patients’ needs rather than visit volume. The role follows a Monday–Friday schedule with no weekends or evenings and no holidays or hospital rounds. Call responsibilities are minimal. Compensation is salaried and includes a strong annual quality-based bonus. HarmonyCares is a national provider of in-home primary care for people with complex healthcare needs, using an integrated, team-based, physician-driven model.
Requirements
- Be licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state where the HarmonyCares Medical Group office is located.
- Hold or be eligible for a federal DEA license.
- Maintain active CPR/BLS certification.
- Hold board certification.
- Maintain a valid driver’s license and a good driving record.
