Advance Practice Provider (NP/ PA) - Value-based Geriatric Primary Care
About The Position
HarmonyCares is seeking an advanced practice provider (NP or PA) to deliver value-based in-home primary care to a high-risk geriatric patient panel. The provider manages chronic conditions and cares for high-acuity patients in their homes, with a focus on patient care rather than visit volume. Clinicians are encouraged to take the time needed to address patients’ needs and work within an integrated, team-based, physician-driven model. The schedule is Monday through Friday, with no weekends or evenings. The workday typically begins around 8:00 a.m. and ends around 4:30 or 5:00 p.m. Athena EMR and Marvix AI Scribe are used to support documentation during the workday. There are no holidays or hospital rounds, and call responsibilities are minimal. Appointment support and a company car with a driver are available in select markets.
Requirements
- Be licensed as a nurse practitioner or physician assistant in the state where the HarmonyCares Medical Group office is located.
- Have a federal DEA license or eligibility for one.
- Hold active CPR/BLS certification.
- Be board certified.
- Have a valid, unencumbered driver’s license and a good driving record.

- Directly related experience in home health, geriatrics, hospice, or primary care.
