About The Position

HarmonyCares is a national value-based provider of in-home primary care for people with complex healthcare needs. The company operates home-based practices in 14 states and employs more than 200 primary care providers. Its care is patient-centered, integrated, team-based, and physician-driven. The role provides primary care to high-acuity patients in their homes, with a focus on taking the time to address patients’ needs rather than seeing patients based on volume. Care is provided Monday through Friday. There are no holidays or hospital rounds, and call responsibilities are minimal. An appointment assistant and a company car with driver are available in select markets.

Requirements

• Licensed as a Nurse Practitioner in the state where the HarmonyCares Medical Group office is located.
• Federal DEA license or eligibility.
• Active CPR/BLS certification.
• Board certification.
• Valid, unencumbered driver’s license and good driving record.

Nice To Haves

• Geriatric training or experience.
• Directly related experience in home health, geriatrics, hospice, and primary care.

Responsibilities

• Provide primary care to high-acuity patients in their homes.
