About The Position

HarmonyCares is a national value-based provider of in-home primary care for people with complex healthcare needs. The organization operates home-based primary care practices in 14 states and employs more than 200 primary care providers who deliver patient-centered care through an integrated, team-based, physician-driven model.

This role provides primary care to high-acuity patients in their homes, with a focus on patient care rather than volume. The position is suited to clinicians with directly related experience in home health, geriatrics, hospice, and primary care who take the time needed to address patients’ needs. The stated schedule is Monday through Friday. There are no holidays or hospital rounds, and call responsibilities are minimal. Appointment support and a company car with a driver are available in select markets.

Requirements

• Licensed as a Nurse Practitioner in the state where the HarmonyCares Medical Group office is located.
• Federal DEA license or eligibility.
• Active CPR/BLS certification.
• Board certification.
• Valid, unencumbered driver’s license and good driving record.

Nice To Haves

• Geriatric training or experience is preferred.
• Directly related experience in home health, geriatrics, hospice, and primary care.
