Advance Practice Provider (NP /PA) Value-based Primary Care
About The Position
HarmonyCares is hiring an advanced practice provider to deliver value-based, in-home primary care to patients with complex healthcare needs, with a focus on geriatric care. The provider manages chronic conditions for their own patient panel and delivers patient-centered care within an integrated, team-based, physician-driven model. The role emphasizes taking the time to address each patient’s needs rather than focusing on volume. The position provides primary care to high-acuity patients in their homes Monday through Friday. There are no weekends, evenings, holidays, or hospital rounds, and call responsibilities are minimal. An appointment assistant and a company car with driver are available in select markets. HarmonyCares is a national provider of in-home primary care services, headquartered in Troy, Michigan, and operating home-based practices in 14 states. Its mission is to bring personalized, quality-based healthcare to the homes of patients who have difficulty accessing care.
Requirements
- Licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state where the HarmonyCares Medical Group office is located.
- Federal DEA license or eligibility for a Federal DEA license.
- Active CPR/BLS certification.
- Board certification.
- Valid driver’s license and good driving record.

- Geriatric training or experience.
- Directly related experience in home health, geriatrics, hospice, and primary care.
Responsibilities
