About The Position

HarmonyCares is hiring an Advance Practice Provider (NP/PA) to deliver value-based, in-home primary care to a high-risk geriatric patient panel. The role focuses on managing chronic conditions and providing patient-centered care to people with complex healthcare needs who have difficulty accessing care. Clinicians are encouraged to take the time needed to address patients’ needs rather than focus on visit volume, working within an integrated, team-based, physician-driven model.

The position follows a Monday–Friday schedule, with no weekends or evenings. The workday generally begins around 8:00 a.m. and ends around 4:30–5:00 p.m. There are no holidays, no hospital rounds, and minimal call responsibilities. Athena EMR and Marvix AI Scribe are used to complete charting during the workday. Appointment support and a company car with a driver are available in select markets.

Requirements

• Licensed as a nurse practitioner or physician assistant in the state where the HarmonyCares Medical Group office is located.
• Federal DEA license or eligibility for a Federal DEA license.
• Active CPR/BLS certification.
• Board certification.
• Valid, unencumbered driver’s license and a good driving record.

Nice To Haves

• Geriatric training or experience is preferred.
• Directly related experience in home health, geriatrics, hospice, and primary care is sought.
