About The Position

HarmonyCares is seeking an Advance Practice Provider (NP or PA) to provide value-based primary care to a high-risk geriatric patient panel in patients’ homes. The role focuses on managing chronic conditions and delivering patient-centered care to people with complex healthcare needs who have difficulty accessing care. Clinicians are encouraged to spend the time needed to address patients’ needs rather than focus on visit volume, working within an integrated, team-based, physician-driven model.

The schedule is Monday through Friday, with no weekends or evenings. The workday generally starts around 8:00 a.m. and ends around 4:30 or 5:00 p.m. There are no holidays or hospital rounds and call responsibilities are minimal. Athena EMR and Marvix AI Scribe are used to support charting during the workday. Appointment support and a company car with a driver are available in select markets.

Requirements

• Licensed as a nurse practitioner (NP) or physician assistant (PA) in the state where the HarmonyCares Medical Group office is located.
• Federal DEA license or eligibility for one.
• Active CPR/BLS certification.
• Board certification.
• Valid, unencumbered driver’s license and a good driving record.

Nice To Haves

• Geriatric training or experience preferred.
• Directly related experience in home health, geriatrics, hospice, and primary care.
