Advance Practice Provider (NP /PA) - Value-based Primary Care
About The Position
HarmonyCares is hiring an advanced practice provider for geriatric, value-based primary care delivered in patients’ homes. The provider manages chronic conditions for an individual patient panel and cares for high-acuity patients as part of an integrated, team-based, physician-driven model. The role emphasizes taking time to address patient needs rather than focusing on visit volume. Charting is completed during the workday using Athena EMR and an AI Scribe. The schedule is Monday through Friday, with no evenings, weekends, or holidays and minimal call responsibilities. An assistant may support appointments in select markets, and a company car with a driver is available in select markets.
Requirements
- Licensed as a Nurse Practitioner in the state where the HarmonyCares Medical Group office is located.
- Federal DEA license or eligibility.
- Active CPR/BLS certification.
- Board certification.
- Valid driver’s license and good driving record.
- Directly related experience in home health, geriatrics, hospice, and primary care is sought.

- Geriatric training or experience preferred.
