About The Position

Requirements

• Registered Nurse (RN) with current and unencu mbered license to practice professional nursing in the state of employment.
• One (1) to two (2) years of recent acute care experience in an institutional setting.
• Possess and maintain valid CPR certification. Must have obtained in-person with a practical skills evaluation as part of the final test.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
• Access to reliable transportation.

Nice To Haves

• Experience in community health education/experience.
• Bachelor's degree in Nursing from an accredited program.
• One (1) year of home health care experience.
• Management experience not required, but direct supervisory experience of Licensed Vocational Nurses and Nurse Aides/Assistants.
• Working knowledge of OASIS documentation requirements.

Responsibilities

• Planning, organizing and directing home care services.
• Building from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
