About The Position

As a CenterWell Home Health nurse reporting to the Clinical Manager, you provide personalized, one-on-one care to help patients regain independence in their homes. You work with physicians and other clinicians to manage care plans that support recovery. The role involves providing skilled nursing care in patients’ homes, monitoring and documenting their care, educating patients and families, and coordinating with healthcare professionals. The position is patient-facing and requires TB screening. It also participates in Humana’s driver safety program and requires a valid state driver’s license, personal vehicle liability insurance, and reliable transportation. The listed pay-per-visit/unit rate is $49.00–$69.00. The stated annual compensation range for full-time employment is $77,200–$106,200, with actual pay varying by location and job-related factors.

Requirements

  • Med-surg, ICU, ER, or acute-care experience.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Experience collaborating with a team of healthcare professionals.
  • Valid driver’s license, auto insurance, and reliable transportation.

Nice To Haves

  • One year of nursing experience.
  • Home Health experience.

Responsibilities

  • Provide skilled nursing care in patients’ homes, including administering medications, performing wound care, and conducting patient assessments.
  • Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
  • Monitor patients’ conditions and report changes.
  • Educate patients and their families about disease management, medications, and treatment options.
  • Maintain accurate records of patient care and coordinate with other healthcare professionals.
  • Report patients’ care and condition progress to their physicians and the Clinical Manager.
  • Oversee Home Health Aides and monitor their completion of documentation.
