About The Position

Collage Home and Community Rehabilitation Partners is seeking experienced Licensed Practical Nurses to provide patient-centered neurological rehabilitation in Denver, Colorado, and surrounding areas. The organization treats people with traumatic brain injuries, spinal cord injuries, CVAs, and other neurological or complex orthopedic diagnoses in their homes and communities. LPNs work with interdisciplinary teams to support individualized treatment plans and provide care in patients’ homes and communities. This position is PRN and pays $34–$36 per hour.

Requirements

• At least 1 year of experience as an LPN in a home care setting.
• Current state license as an LPN.
• Graduate of an accredited practical nursing program.
• Proof of current CPR certification.
• Proof of a current negative TB test.
• Ability to work a flexible schedule and travel locally.
• Excellent interpersonal skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Participate in planning and coordinating total patient care with the RN and in conjunction with the physician’s plan of care, and follow each patient’s nursing care plan.
• Accurately report and record the patient’s condition and care, including signs and symptoms that may indicate a change.
• Prepare clinical and progress notes and summaries.
• Assist patients with activities of daily living while teaching appropriate self-care techniques.
• Provide and maintain a safe environment for the patient.
• Assist the physician and Registered Nurse in complex nursing situations.
• Follow universal precautions when providing patient care.
• Follow emergency procedures in the event of a catastrophic event or incident, such as an accident, injury, or significant change in the patient’s condition.
