Licensed Practical Nurse (LPN) PRN
About The Position
Beacon Health Management is hiring a Licensed Practical Nurse (LPN) to work at Hazlehurst Court Care and Rehab, which provides short-term rehabilitation and long-term care. In conjunction with the Unit Director, the LPN applies a general understanding of nursing principles to develop and implement individualized nursing care plans that meet customers’ needs. The LPN assists with orientation and supervision of nursing personnel, attends to daily unit operations on a per-shift and unit-level basis, and assumes a leadership role as needed. The position involves direct customer care, medication and treatment administration, clinical documentation, care planning, communication with clinical staff and physicians, and supervision and coordination of nursing personnel. The LPN also supports customers’ comfort and safety, assists physicians as needed, attends mandated in-service programs, and helps ensure customers and families receive caring, compassionate service that respects individual needs and rights.
Requirements
- Graduate of an approved school for Practical or Vocational Nursing.
- Currently licensed by the State Board of Nursing.
Nice To Haves
- Experience in geriatric or rehabilitative nursing is preferred.
Responsibilities
- Take an active role in direct customer care.
- Administer medications and treatments and document them.
- Complete clinical documentation related to customer activity.
- Assist in developing and implementing an individual nursing care plan for each customer.
- Participate in care plan meetings.
- Count and record controlled drugs at the beginning and end of each shift.
- Communicate pertinent data to the Unit Manager/Supervisor and/or Physician.
- Review and update the communication board, and monitor and address alerts throughout the shift.
- Provide for customers’ emotional and physical comfort and safety.
- Assist the Physician with rounds and customer examinations as needed.
- Supervise and coordinate Certified Nursing personnel and other LPNs in providing direct customer care in accordance with State, Federal, and Corporate guidelines.
- Attend all mandated in-service programs.
- Ensure customers and families receive high-quality, caring, and compassionate service that recognizes individual needs and rights.
