About The Position

Requirements

• Current Nurse Practitioner certification.
• Active New York RN license and Nurse Practitioner certification.
• Active DEA registration for prescribing controlled substances.
• Experience in subacute rehabilitation, geriatrics, orthopedics, internal medicine, pain management, or a related setting.
• Strong clinical assessment, documentation, and communication skills.
• Ability to manage a patient panel and collaborate effectively with interdisciplinary teams.

Responsibilities

• Perform comprehensive patient assessments and follow-up visits in subacute rehabilitation settings.
• Manage rehabilitation-related medical needs, pain, and conditions affecting mobility and function.
• Develop individualized treatment plans in collaboration with facility clinical teams and physicians, as applicable.
• Prescribe and manage medications within the scope of practice.
• Perform joint and trigger-point injections when appropriately trained and authorized.
• Coordinate with physical, occupational, and speech therapy teams to support rehabilitation goals.
• Complete accurate, timely documentation of treatment plans and patient progress.
• Educate patients and families and support care transitions.
