About The Position

Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations for health plan members. Clinicians build trusted relationships, help connect people with appropriate care, and provide flexible, comprehensive, and personalized evaluations in members’ homes. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change a member’s current treatment regimen.

The role includes locally and in-state travel as needed, as well as virtual visits when needed. The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants; candidates must be able to perform duties comfortably and safely in these conditions.

The position is part-time and per diem, with 12 anticipated weekly hours. Compensation includes an hourly base rate and may include per-visit and other incentives. Malpractice insurance coverage applies during Signify Health-related activities, and supplies and other perks are provided.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
• Build trusted relationships with health plan members and help connect them to appropriate care.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Fulfill the requirements of the specified clinician role and provide Signify Health services as indicated.
