About The Position

Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations and help bridge gaps in care. Clinicians visit members in their homes for approximately 45 minutes to connect one-on-one, answer health questions, review medication and medical history, and conduct a physical evaluation. Depending on the member’s health plan, visits may also include diagnostic tests such as diabetic eye exams, spirometry, or bone density screening. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change a member’s current treatment regimen.

The role involves local travel and travel within licensed states as needed, as well as virtual visits when needed. The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants; clinicians must be able to perform their duties comfortably and safely in those conditions.

This part-time, per diem position has an anticipated schedule of 12 hours per week. Per diem employees receive an hourly base rate with additional incentives potentially available, malpractice insurance coverage during Signify Health-related activities, supplies, and other perks. The position is eligible for certain benefits and programs, depending on hours worked and eligibility, including dental and vision coverage, wellness resources, employee discounts, and access to certain voluntary benefits.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
