In-Home Nurse Practitioner or Physician Assistant Per Diem - Reno
About The Position
Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations to health plan members in the Reno area. The role focuses on building trusted relationships and helping connect people with care suited to their needs. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a person’s current treatment regimen. Virtual visits may also be conducted as needed. The primary workplace is members’ private residences. Home environments vary and may contain household pets, secondhand smoke, or other airborne allergens or irritants. Clinicians must be able to perform their duties comfortably and safely in these conditions. The position requires local travel and travel within licensed states as needed.
Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.
- Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Perform additional diagnostic tests when ordered by a member’s health plan, including services such as diabetic eye exams, spirometry, and bone density screening.
- Conduct virtual visits as needed.
- Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
- Travel locally and within licensed states as needed.
- Engage with members, answer their health questions, and help connect them with appropriate care.
