About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations to health plan members in Reno. Clinicians visit members in their homes to build trusted relationships, help identify gaps in care, and support better health outcomes. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a member’s current treatment regimen. The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. The role may also involve virtual visits and local travel, including travel within licensed states as needed.

Requirements

Active, unrestricted license(s) in the coverage area(s).
Board certification.
Ability to comfortably and safely perform duties in diverse home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
Answer members’ health questions and provide education during visits.
Perform additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
Conduct virtual visits as needed.
Travel locally and within licensed states as needed.
Fulfill the requirements of the specified Signify Health clinician role and contribute to efforts to improve health outcomes and connect members to appropriate care.
