In-Home Nurse Practitioner or Physician Assistant (Full Time) - Newport News
About The Position
Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations for health plan members. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and a clinical workflow. They are not required to prescribe medication, order lab tests, or change members’ current treatment regimens. The role may also involve virtual visits and locally or multi-state travel as needed. The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, or other airborne allergens or irritants. Clinicians must be able to perform their duties comfortably and safely in these conditions.
Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.

Responsibilities
- Conduct in-home health evaluations and build trusted relationships with health plan members.
- Review members’ medications and medical histories and perform physical evaluations.
- Provide additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, or bone density screening.
- Conduct virtual visits as needed.
- Travel locally and within licensed states as needed.
- Fulfill the requirements of the clinician role and provide Signify Health services as indicated.
