About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to conduct in-home health evaluations for health plan members. The role focuses on building trusted relationships, helping identify care gaps, and providing flexible, comprehensive, and personalized health evaluations in members’ homes. Clinicians may also conduct virtual visits as needed and travel locally and within licensed states as required. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan, such as diabetic eye exams or spirometry. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change members’ current treatment regimens. The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be able to perform their duties comfortably and safely in these conditions.

Requirements

Active, unrestricted license(s) in the coverage area(s).
Board certification.

Responsibilities
