In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Saginaw, MI
About The Position
Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations and help bridge gaps in care. Clinicians meet with health plan members in their homes to build trusted relationships and provide flexible, comprehensive, personalized evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the person’s health plan. Clinicians use an iPad and clinical workflow and are not required to prescribe medication, order lab tests, or change a member’s treatment regimen. Virtual visits may also be conducted as needed. The primary workplace is the private residence of health plan members. Home environments vary and may contain household pets, secondhand smoke, airborne allergens, or irritants; candidates must be able to perform their duties comfortably and safely in these conditions. The role requires flexibility to travel locally and within licensed states as needed. The position is 40 hours per week and offers a comprehensive benefits package to eligible full-time employees.
Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.
- Ability to comfortably and safely perform duties in varied home environments, including homes with household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Perform additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
- Use an iPad and the clinical workflow to complete evaluations.
- Conduct virtual visits as needed.
- Travel locally and within licensed states as needed.
- Engage with health plan members, answer health questions, provide education, and help connect people with appropriate care.
