About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations and help bridge gaps in care. Clinicians visit health plan members in their homes, build trusted relationships, and provide flexible, comprehensive, personalized evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the person’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change a person’s current treatment regimen. The role may include virtual visits and local travel within licensed states. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. Candidates must be able to safely and comfortably perform their duties in these environments. The position is full time, with 40 anticipated weekly hours. The typical pay range is $95,738.00–$206,206.00; actual pay depends on factors including experience, education, and geography.

Requirements

Active, unrestricted license(s) in coverage area(s).
Board certification.

Responsibilities

Conduct in-home health evaluations and build trusted relationships with health plan members.
Review members’ medication and medical history and perform a physical evaluation.
Perform additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
Conduct virtual visits as needed.
Travel locally and within licensed states as needed.
Fulfill the requirements of the specified clinician role and provide Signify Health services as indicated.
