About The Position

Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations in Youngstown. Clinicians engage with people in their homes, build trusted relationships, help bridge gaps in care, and connect members to care suited to their needs.

Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by a member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a person’s current treatment regimen. Virtual visits may also be conducted as needed.

The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, or other airborne allergens or irritants. The role requires clinicians to perform duties comfortably and safely in these conditions and to travel locally and within licensed states as needed.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in members’ homes, which may contain pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Provide Signify Health services as indicated and help connect members with care suited to their needs.
