About The Position

Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations. Clinicians engage with people in their homes, build trusted relationships, and help bridge gaps in care by providing flexible, comprehensive, and personalized health evaluations.

Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and—when ordered by the person’s health plan—additional diagnostic tests such as diabetic eye exams or spirometry. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a person’s current treatment regimen. The role may also include virtual visits and local travel within licensed states. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants; clinicians must be able to perform their duties comfortably and safely in these environments.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
• Use an iPad and clinical workflow to complete evaluations.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Provide education, answer health questions, and help members connect with care suited to their needs.
• Fulfill the requirements of the specified clinician role and provide Signify Health services as indicated.
