About The Position

Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations in Des Moines. The role engages health plan members in their homes, helps bridge gaps in care, and builds trusted relationships while providing flexible, comprehensive, and personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and a clinical workflow. They are not required to prescribe medicine, order lab tests, or change a member’s current treatment regimen.

The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. The position may also involve virtual visits and local travel, including travel within licensed states as needed. Per diem compensation includes an hourly base rate and may include per-visit and other incentives. Malpractice coverage applies during Signify Health-related activities; supplies and other perks are also provided.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by the member’s health plan, such as diabetic eye exams, spirometry, or bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Build trusted relationships with members and help connect them with care suited to their needs.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
