About The Position

Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations. Clinicians visit health plan members in their homes to build trusted relationships, help bridge gaps in care, and connect people with care suited to their needs. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, time for questions, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen.

The position involves local travel and travel within licensed states as needed, as well as virtual visits when needed. The primary workplace is members’ private residences, where conditions vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. Clinicians must be able to perform their duties comfortably and safely in these environments.

The role is part-time and per diem, with an anticipated 12 hours per week. Compensation includes an hourly base rate and may include additional incentives, such as payment per completed visit. Malpractice coverage applies during Signify Health-related activities. Supplies and other perks are also provided. The listed pay range is $46.03–$99.14 per hour, with the actual offer depending on factors including experience, education, and geography.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct In-Home Health Evaluations, including medication and medical history reviews and physical evaluations.
• Answer members’ health questions and help connect them with appropriate care.
• Perform additional diagnostic tests when ordered by a member’s health plan, including services such as diabetic eye exams, spirometry, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
• Work as a clinician colleague in accordance with the requirements of the role.
