About The Position

Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations to health plan members in Canton. The clinician conducts visits of approximately 45 minutes in members’ homes, building trusted relationships and helping bridge gaps in care. Visits include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order laboratory tests, or change members’ current treatment regimens. Virtual visits may also be conducted as needed.

The role requires local travel and travel within licensed states as needed, along with providing Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. The position is part-time and per diem, with an anticipated 12 hours per week. Compensation includes an hourly base rate and may include incentives. Malpractice coverage applies during Signify Health-related activities.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.

Responsibilities
