In-Home Nurse Practitioner or Physician Assistant Full Time - Canton
About The Position
Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations and help bridge gaps in care. Clinicians visit health plan members in their homes to build trusted relationships and provide flexible, comprehensive, and personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and any additional diagnostic tests ordered by the member’s health plan. Clinicians use an iPad and clinical workflow and are not required to prescribe medicine, order lab tests, or alter a person’s current treatment regimen. Virtual visits may also be conducted as needed. The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other potential airborne allergens or irritants. The position requires local travel and travel within licensed states as needed.
Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.
- Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Provide additional diagnostic and preventive services as indicated, such as spirometry, diabetic eye exams, and bone density screening.
- Build trusted relationships with health plan members and help connect them with appropriate care.
- Conduct virtual visits as needed.
- Travel locally and within licensed states as needed.
- Fulfill the requirements of the specified clinician role and provide Signify Health services as indicated.
