In-Home Nurse Practitioner or Physician Assistant Full Time - Akron
About The Position
Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations and help bridge gaps in care. Clinicians visit health plan members in their homes, building trusted relationships and providing flexible, comprehensive, personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change members’ current treatment regimens. The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. Clinicians must be able to perform their duties comfortably and safely in these settings. Local and in-state travel is required as needed, and virtual visits may also be conducted. This is a full-time position with 40 anticipated weekly hours. The listed pay range is $87,035 to $187,460. The actual offer depends on factors including experience, education, geography, and other relevant factors.
Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.
- Ability to perform duties comfortably and safely in home environments that may contain household pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities
- Conduct In-Home Health Evaluations and build trusted relationships with health plan members.
- Review members’ medications and medical histories and perform physical evaluations.
- Provide additional diagnostic tests when ordered by the member’s health plan, including services such as diabetic eye exams, spirometry, and bone density screening.
- Use an iPad and the clinical workflow for each evaluation.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
- Help connect members with care suited to their individual needs.
