In-Home Nurse Practitioner or Physician Assistant Full Time - Louisville
About The Position
Signify Health, part of CVS Health, is seeking a full-time clinician to provide in-home health evaluations to health plan members and help bridge gaps in care. Visits take about 45 minutes and include a medication and medical history review, a physical evaluation, and, when ordered by the member’s health plan, additional diagnostic tests such as diabetic eye exams or spirometry. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen. The clinician will build trusted relationships with members, support improved health outcomes, and connect people with care suited to their needs. The role involves local travel and travel within licensed states as needed, virtual visits when needed, and delivery of Signify Health services, including diagnostic and preventive services. The primary workplace is members’ private residences. Homes may contain pets, secondhand smoke, or other airborne allergens or irritants, and clinicians must be able to perform their duties comfortably and safely in these environments.
Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.
- Ability to comfortably and safely perform duties in homes that may contain pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities
