In-Home Physician Full Time - Louisville
About The Position
Signify Health, part of CVS Health, is seeking a full-time Physician to provide in-home health evaluations and help bridge gaps in care. The role involves building trusted relationships and delivering flexible, comprehensive, and personalized evaluations in health plan members’ homes. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and any additional diagnostic tests ordered by the person’s health plan. Physicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a person’s current treatment regimen. The role may also include virtual visits and local travel, as well as travel within licensed states as needed. The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be able to comfortably and safely perform their duties in these conditions. The position is full-time, with 40 anticipated weekly hours. The stated pay range is $125.00–$125.00 per hour. Full-time employees are eligible for a comprehensive benefits package, subject to eligibility.
Requirements
- Hold an active, unrestricted physician license in the coverage area(s).
- Be able to comfortably and safely perform duties in home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Perform additional diagnostic tests when ordered by a person’s health plan, including services such as spirometry, diabetic eye exams, and bone density screening.
- Use an iPad and the clinical workflow for each evaluation.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
- Help connect people to care suited to their needs and support improved health outcomes.
