In-Home Physician Full Time - Champaign
About The Position
Signify Health, part of CVS Health, is seeking a full-time physician to provide in-home health evaluations and help bridge gaps in care. Physicians visit people in their homes to build trusted relationships and provide flexible, comprehensive, personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and—when ordered by the person’s health plan—additional diagnostic tests such as diabetic eye exams or spirometry. Physicians use an iPad and a clinical workflow and are not required to prescribe medicine, order lab tests, or alter a person’s current treatment regimen. The role involves local travel and travel within licensed states as needed, virtual visits as needed, and delivery of indicated Signify Health services, including diagnostic and preventive services. The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants; candidates must be able to perform their duties comfortably and safely in these conditions.
Requirements
- Active, unrestricted physician license(s) in the coverage area(s).
- Ability to comfortably and safely perform duties in home environments that may contain household pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Perform additional diagnostic tests when ordered by a person’s health plan, including services such as diabetic eye exams and spirometry.
- Use an iPad and the clinical workflow for each evaluation.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Provide indicated Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening.
- Help connect people with appropriate care and make referrals to their health plans when applicable.
- Work as a clinician colleague and fulfill the requirements of the specified role.
