About The Position

Signify Health, part of CVS Health, is seeking a per diem physician to provide in-home health evaluations and help bridge gaps in care. Visits last approximately 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Physicians use an iPad and an intuitive clinical workflow. The role does not require prescribing medication, ordering lab tests, or changing a person’s current treatment regimen.

The physician joins a national network of clinicians focused on improving health outcomes and helping people connect with care suited to their needs. The role may include local travel and travel within licensed states, as well as virtual visits as needed. Physicians provide Signify Health services as indicated, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening.

The primary workplace is health plan members’ private residences. Home environments vary and may include household pets, secondhand smoke, or other airborne allergens or irritants; clinicians must be able to perform their duties comfortably and safely in these settings. Per diem employees receive an hourly base rate and may qualify for additional incentives, along with malpractice coverage during Signify Health-related activities and supplies and other perks.

Requirements

• Active, unrestricted physician license(s) in the coverage area(s).
• Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by a member’s health plan.
• Use an iPad and clinical workflow to complete each evaluation.
• Help connect people to care suited to their needs and support improved health outcomes.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide indicated Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening.
• Work as a clinician colleague and fulfill the requirements of the specified role.
