In-Home Physician Per Diem - Columbia
About The Position
Signify Health, part of CVS Health, is seeking a per diem Physician to provide In-Home Health Evaluations in members’ homes and help bridge gaps in care. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Physicians use an iPad and an intuitive clinical workflow. The role does not require prescribing medication, ordering laboratory tests, or changing a person’s current treatment regimen. The position involves building trusted relationships, helping people connect with appropriate care, and providing personalized health evaluations. Physicians may travel locally and within licensed states, conduct virtual visits as needed, and provide Signify Health services including diagnostic and preventive services. The primary workplace is members’ private residences, which may have household pets, secondhand smoke, or other airborne allergens or irritants. The role is part time, with anticipated weekly hours of 12.
Requirements
- Active, unrestricted physician license(s) in the coverage area(s).
- Ability to safely and comfortably perform duties in members’ homes, which may contain household pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Perform additional diagnostic tests when ordered by a person’s health plan.
- Use an iPad and clinical workflow to complete each evaluation.
- Help connect people to appropriate care and support improved health outcomes.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening.
