About The Position

Signify Health, part of CVS Health, is seeking a per diem physician to provide in-home health evaluations and help bridge gaps in care. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the health plan. Physicians use an iPad and a clinical workflow and are not required to prescribe medication, order lab tests, or change a person’s current treatment regimen. The role may also involve virtual visits and providing diagnostic and preventive services. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Requirements

• Active, unrestricted medical license(s) in the coverage area(s).
• Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations and build trusted relationships with health plan members.
• Review each member’s medication and medical history and perform a physical evaluation.
• Provide additional diagnostic tests when ordered by the member’s health plan.
• Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Work as a clinician colleague and fulfill the requirements of the specified role.
