About The Position

Signify Health, part of CVS Health, is seeking a per diem physician to provide in-home health evaluations to health plan members in Milwaukee and other locations within licensed states as needed. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and any additional diagnostic tests ordered by the member’s health plan. Physicians use an iPad and a clinical workflow, and are not required to prescribe medicine, order lab tests, or change a member’s current treatment regimen. Virtual visits may also be conducted as needed. The role focuses on building trusted relationships, improving health outcomes, connecting people with appropriate care, and providing personalized evaluations in members’ homes. The primary workplace is members’ private residences, where conditions may vary and may include household pets, secondhand smoke, or other airborne allergens or irritants. The position is part-time, per diem, with 12 anticipated weekly hours.

Requirements

• Hold active, unrestricted physician license(s) in the coverage area(s).
• Be able to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic and preventive services as indicated, such as spirometry, diabetic eye exams, and bone density screening.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Build trusted relationships with members and help connect them with care suited to their needs.
• Work as a clinician colleague in accordance with Signify Health role requirements.
